Sorry, don't know where you can get this sticker.......but my missus used to have it many years ago on the back of her XR3i. lol.

I have heard (from another post on another web site) that there are quite a few stickers on white van's now with the usual "hows my driving" type of thing.

The only problem is that the phone numbers are adult chat lines !LOL

Go to Staples and buy Inkjet Printer Sticker sheets and print one yourself.

I really don't know where they sell those stickers. Instead of you trying to find where, I would suggest you go to a sticker maker and have yourself a customized one. That way, your sticker would look better and just the way you want it to be.
